A statement that the information in the notification is accurate, and under penalty of perjury, that the complaining party is authorized to act on behalf of the copyright owner.

Section 512(f) of the US Copyright Law provides civil penalties, including costs and attorneys' fees, against anyone who identifiably and materially provides certain information in a notification of infringement under Section 512(c)(3) of 17 USC.

Reply Notice - Retrieval of Materials

If you have received a takedown notice for alleged copyright infringement, you may submit a response notice with efforts to restore the material in question to the Site. Notice thereof must be given in writing to our DMCA Agent and must contain substantially similar elements in accordance with Section 512(g)(3) of 17 USC:

Your physical or electronic signature.
A description of the material that has been removed and its original location before it was removed.
A statement under sworn statement that you have a good faith belief that the material has been removed or restored

Its length as a result of an error or misidentification of the material that must be removed or disabled.
Your name, address, and telephone number, and a statement that you consent to the jurisdiction of a federal court for the judicial district in which the address is located (or, if you are outside the United States, that you consent to the jurisdiction of any judicial district in which the service provider may be found), and that you will accept service of process from the person or company Which provided the original notification of copyright infringement.

Repeat copyright infringement policy

We take copyright infringement very seriously. In accordance with the requirements of the Digital Copyright Act Repeat Infringement Policy, we maintain a list of DMCA notices from copyright holders and make a good faith effort to identify any repeat infringers. The accounts of those who violate our repeated copyright infringement policy will have their accounts terminated.
